Click on the "Surveys" tab located at the top of the administrative user interface.

Select the Survey you want to edit in the list of Surveys below the "Surveys" small tab located on the left hand side of the administrative user interface.

Click on the "Edit the selected survey." button.

In the Survey window, click on the "Actions" tab.

In the grid of survey actions, click on the "Advanced Settings" tab then click on the survey action you want to modify.

Enter the name of the survey action.

Enter an email address, or a URL, or both depending on the type of survey action you want to create.

Select the conditions that should trigger this action.

Click on the "Save" button.
